Case Management Services
About the Service
Case management is provided through a grant from the Care Coordination for Elderly Virginians Program. This service supports Loudoun County residents age 60 and older who are at risk of nursing facility placement, or have experienced a recent health care crisis and need help with at least two activities of daily living, such as bathing, dressing, eating, transferring, or toileting. A case manager can assist an older adult to develop a plan of care based on an assessment of care and service needs.
Services Provided
Case Management provides:
- Arrangement of service delivery
- Care plan development
- Comprehensive in-home assessment
- Initial eligibility screening
- On-going monitoring of client’s status and delivery of services

- Re-assessment of client's needs over time
- Referral to other eligible programs
- Short-term service coordination
- Supportive counseling

Who May Need Case Management
Residents that may need Case Management include:
- Older adults who require coordination of multiple services
- Older adults whose family or friends live outside of the area, or who are unable to coordinate and oversee services
- Older adults living alone and unable to access services without help
