How Service Coordination Works
All families involved with the Infant and Toddler Connection of Loudoun receive service coordination, a process that connects families with other community resources, supports, and services including:

• Child care
• Federal medical and subsidy programs
• Knowledge and understanding of medical diagnoses
• Communication with medical personnel
• Coordination with other service providers

Services
Each child and family has an Individualized Family Service Plan (IFSP), which is unique to each family. Services include:

• Developmental Services
• Occupational Therapy
• Physical Therapy
• Speech and Language Therapy
• Service Coordination
