Disease prevention and health promotion services provide educational opportunities and information about prevention, treatment, and reduction of the effects of chronic disabling conditions to adults age 60 and older. Information about promoting healthy lifestyles is also provided. The goal of the program is to empower older adults to improve health and prevent disease by taking more control over their own health through lifestyle and behavioral changes.
Services focus on providing information and programs that enhance opportunities for older adults to learn about and take responsibility for their own health. Educational presentations are offered on a variety of health-related topics, such as comprehensive fitness, nutrition, mental health, and safety.
The Chronic Disease Self-Management Program is a workshop lasting two and one-half hours, which is offered once a week for six weeks in community settings (e.g., senior centers, churches, libraries, hospitals). People with different chronic health problems attend together. Workshops are facilitated by two trained leaders.
