Educational opportunities and information about prevention, treatment, and reduction of the effects of chronic conditions to adults age 60 and older. Information about promoting healthy lifestyles is also provided.
Live Well with Chronic Diseases and Live Well with Diabetes programs
Chronic Disease Self-Management Education offers workshops lasting two and a half hours, once a week for six weeks in community settings (e.g., senior centers, churches, libraries, hospitals). People with different chronic health problems attend together. Workshops are facilitated by two trained leaders.
- Meal planning and healthy eating
- Medication management
- Dealing with difficult emotions
- How to prevent falls
- Glucose monitoring
