Form 1095-C for your 2017 Tax Reporting (2/26/18)

If you are a retiree or a dependent enrolled in one of the county-sponsored pre-65 health plans will receive a Form 1095-C. The Affordable Care Act requires employers to report the value of employees' health care coverage provided under an employer-sponsored group health plan annually.  The original deadline forms needed to be mailed was January 31, 2018.  In December, the Internal Revenue Service granted an extension to all employers to March 2, 2018. You can expect to receive your form by US Mail no later than the March 2nd deadline. The county contracts with an outside vendor, ACA TRACK, to provide services related to this mandate including printing of the Form 1095 and IRS reporting. If you have questions, call their Customer Service Hotline at 1-877-255-3591 . Staff is available. You can also find more information on the IRS website.

An Ounce of Prevention
All of the Retiree Health Plans feature in-network preventive care benefits at no cost to you.  Identify risks before they become chronic.  Early detection helps to mitigate more serious medical conditions later.  Get your Annual Preventive Exams as recommended.

Care When You Need It
Telehealth Services through Cigna provide access to board-certified doctors & pediatricians 24/7 for non-urgent health care - just a phone call or video chat away!  Get the appropriate level of care when you need it!


What are My Vision Benefits? 
That answer depends on if you are eligible or not eligible for Medicare.  For all Pre-65 retirees and their eligible dependents in the Cigna Point of Service or Cigna Open Access Plus health plans, your vision benefit is with Davis Vision.  Comprehensive annual eye exam is covered at 100% after a $15 copay with an in-network provider.  For a complete summary of the plan, click here.  Provide your Davis Vision member ID card to your vision provider.  

If you or eligible dependents are enrolled in the Post-65 Cigna Medicare Surround plan, then your vision benefits are included in your Cigna Medicare Surround plan.  Comprehensive annual eye exam is covered at 100% after a $30 copay.  For a complete summary of the vision benefits, click here.  Provide your Cigna Medicare Surround Insurance card to your vision provider.  Please remember to provide your vision provider with your new insurance card if you transition from the Pre-65 to the Post 65 Cigna Medicare Surround Plan.  This ensures claims are processed accurately and timely.
