Filing Process
Step One: Select and complete the certificate of assumed or fictitious name you wish to file:

Step Two: Determine if you wish to submit your certificate in person ("Submit Online") or by mail ("Non-Local Mail in Option").

Please note, the option to submit online is currently unavailable. Please complete and print out the form you wish to file. You can then choose to bring the completed form to the Clerk's Office in person or mail in the completed and notarized form (see below). 

In Person
Customers submitting their certificate in person at the Clerk’s office must bring a valid photo identification and sign the form in front of the Deputy Clerk. In the case of partnerships, all partners listed on the certificate must appear together to sign the certificate. 

Mail In
Customers that choose the mail-in option must have their signature notarized in front of a Notary Public prior to mailing. In the case of partnerships, all partners listed on the certificate must appear and have their signature notarized in front of a Notary Public prior to mailing. Once completed and notarized, mail the certificate, payment (check or money order) and a self-addressed stamped envelope for return of your receipt to:

Fees
Certificate of Assumed or Fictitious Name filing fee is $10.00;
Cancellation of Certificate of Assumed or Fictitious Name filing fee is $10.00;
Copies are $.50 per page and additional $2 for each certified copy.

Certificate of Assumed or Fictitious Name filed at the local level does not have the same status as those filed with the State Corporation Commission.
You are strongly encouraged to consult with an attorney concerning the protection of your business assumed or fictitious name.            

To avoid duplication of assumed or fictitious business name, it is recommended that the owner reviews the Business Name Index on the Public Access System in the Circuit Court Clerk’s office.
