Workers' Compensation
Promptly report injury / illness to your immediate supervisor or designated person in accordance with your department's protocol within 24 hours.  To report a work-related incident you must complete the Employee's Report of Injury along with your supervisor.  The Employee's Report of Injury can be found on page 9 of the Workers' Compensation Accident Reporting Packet .

For all work-related injuries / illnesses, you must choose a physician from the 
Panel of Physicians.

Medical Treatment
You will need to take a Medical Treatment - Physical Demands Analysis (PDA) form with you to each medical appointment for your treating physician to complete.  You must provide a copy to HealthSmart within 24 hours of your appointment.  
  • Treatment referrals must be authorized by HealthSmart in advance of your actual appointment.
  • Referrals and/or follow-up visits must be scheduled within 2 business days.
  • You must take the first available appointment offered to you.  If you have difficultly obtaining an appointment, contact HealthSmart immediately.

You must present the HealthSmart Casualty Claims billing contact information to the medical provider / facility to insure the correct billing.  Failure to do so may result in you receiving medical bills directly.

Family & Medical Leave
Family & Medical Leave is job protected leave for eligible employees with certain medical conditions including work-related injuries and illnesses.  FMLA runs concurrent with Workers' Compensation and Injury Leave.
