Re-Entry Resources: Physical Health and Nutrition
The Loudoun County Re-Entry Advisory Council provides information about some of the community resources available in Loudoun that can help ex-offenders address the challenges they face after returning to the community following incarceration. 

Physical Health
Loudoun County Health Department
The Community Health Services Division of the Loudoun County Health Department provides a variety of services, including family planning, maternity, immunizations to help protect the public from vaccine-preventable diseases and testing for tuberculosis, sexually-transmitted diseases, and HIV.  The division provides follow-up of all reportable communicable diseases and also provides information on illness prevention, wellness, chronic diseases, and many other health-related subjects. The Health Department's primary medical and dental clinics are located at:

Loudoun Free Clinic
The Loudoun Free Clinic at 224-A Cornwall Street, N.W. in Leesburg, provides medical care to people struggling with emergency, acute and chronic medical conditions, at no cost to the patients. Patients should call the clinic's Eligibility Coordinator at 703-779-5424 to schedule an appointment.

HealthWorks  
HealthWorks provides affordable medical, dental and behavioral health care to anyone, including those who are uninsured or under-insured. HealthWorks also provides eligible patients with access to a Prescription Assistance Program.

Nutrition and Food Assistance
Supplemental Nutrition Assistance Program (SNAP) |Benefits
SNAP offers nutrition assistance to millions of eligible, low-income individuals and families. Applications for SNAP benefits may be filed online.

Loudoun Hunger Relief

Loudoun Hunger Relief provides services to those living in Loudoun County who are struggling to make ends meet. The organization provides provide fresh groceries for the whole family, plus can help find further assistance to meet other needs through its network of partners.
